Indications 

Tecfidera is a prescribed medication indicated for the treatment of relapsing forms of multiple sclerosis (MS) in adult patients, encompassing clinically isolated syndrome, relapsing-remitting disease, and active secondary progressive disease. 

Tecfidera falls within the category of selective immunosuppressants. Its precise mode of action in treating multiple sclerosis remains unknown, though it is thought to mitigate inflammation and forestall nerve damage that can lead to multiple sclerosis symptoms. 

Uses and Dosage 

Before taking Tecfidera and with every prescription refill, carefully review its medication guide and patient information leaflet. Should you have any queries, consult your doctor or pharmacist. 

Your doctor will initiate treatment with a low dosage of Tecfidera and gradually escalate it after one week. 

Typically, this medication is taken twice daily as prescribed by the doctor.  

While it can be taken with or without food, consuming it with a meal or a non-enteric coated aspirin (325 mg or less) half an hour beforehand might mitigate flushing—a sudden sensation of warmth, redness, or tingling. 

Tecfidera capsules must be swallowed whole. Avoid crushing, chewing, or dispersing their contents onto food, as this can lead to a sudden release of the entire medication, potentially elevating the likelihood of adverse effects. 

To achieve the best outcome, take this medication exactly as directed the same times daily. Refrain from modifying the recommended dosage, frequency, or duration of use without seeking guidance from your doctor. 

Side Effects

Common Side Effects 

• Stomach pain, indigestion, nausea, vomiting, diarrhea 
• Redness, rash, itching 
• Flushing 

Serious Side Effects 

• Herpes virus symptoms:  
  • Flu-like symptoms 
  • Cold sores near mouth 
  • Rash with tingling or pain 
  • Burning pain in the thigh or lower back 
• Low white blood cell counts: 
  • Fever 
  • Mouth sores 
  • Skin sores 
  • Sore throat 
  • Cough 
  • Difficulty breathing 
• Liver problems:  
  • Reduced appetite 
  • Upper right stomach pain 
  • Fatigue 
  • Itching 
  • Dark urine 
  • Pale stools 
  • Jaundice (yellowing of skin or eyes) 

Call your doctor right away if any of the serious side effects above are experienced. 

Immediate medical attention should be sought if any issues arise regarding speech, cognition, vision, or muscle control while taking Tecfidera, as it may lead to a grave brain infection with potentially life-altering outcomes. These symptoms might manifest gradually and deteriorate rapidly. 

Allergic Reactions 

Although rare, a severe allergic reaction to Tecfidera may occur. Promptly seek medical attention in case of any signs of an allergic reaction, such as: 

• Hives 
• Difficulty breathing 
• Face, lips, throat, or tongue swelling

Cautions 

Before starting treatment with Tecfidera, inform your doctor if you have any known allergies, particularly to dimethyl fumarate, diroximel fumarate, or monomethyl fumarate. 

Consult your pharmacist for more information about Tecfidera, as it may contain inactive ingredients that may trigger allergic reactions or other complications. 

To ensure Tecfidera is safe for you, disclose to your physician your medical background, especially if you have a history of: 

• Low white blood cell counts 
• Infections (including any existing infection) 
• Liver disease 

Certain products might interact with Tecfidera, including other medications that suppress the immune system or elevate infection risk (for instance, natalizumab, rituximab). 

To minimize the risk of such drug interactions, notify all healthcare providers of any current or recently started or stopped medications, including prescription and over-the-counter drugs, vitamins, and herbal supplements. 

Dimethyl fumarate closely resembles diroximel fumarate and monomethyl fumarate. Refrain from using medications containing diroximel fumarate or monomethyl fumarate while taking Tecfidera. 

Tecfidera can elevate infection risk or exacerbate ongoing infections. Avoid contact with individuals who have highly transmissible infections like chickenpox, COVID-19, measles, or the flu. For exposure to infections or further information, consult your doctor. 

Inform your healthcare provider about your use of Tecfidera prior to receiving any immunizations. Refrain from being in close proximity to individuals who have recently been administered live vaccines, such as the nasal flu vaccine. 

Tecfidera should only be used when necessary during pregnancy. Consult your doctor to evaluate the potential risks and benefits. 

The transfer of dimethyl fumarate into breast milk is uncertain. Prior to breastfeeding, seek advice from your doctor.
