Indications 

Lotemax is an ophthalmic suspension medication utilized for the treatment of steroid-responsive allergic and inflammatory conditions affecting the conjunctiva, which is the outer membrane of the eye.  

Lotemax is also used to manage inflammation resulting from eye surgery. 

Classified as a corticosteroid, Lotemax exerts its therapeutic effects by inhibiting the release of the body’s natural substance called lipocortins, known to induce swelling, itching, and pain. 

Uses and Dosage 

Before using Lotemax and every time you get a refill, thoroughly review its medication guide and patient information leaflet. If you have questions or concerns, do not hesitate to seek advice from your physician or pharmacist. 

The dosage and length of treatment with Lotemax are based on the individual’s medical condition and response to treatment. If given a different prescription than listed here, adhere to it strictly. 

For steroid-responsive disease treatment:  

• 1-2 drops of Lotemax are recommended to be administered to the affected eye(s) four times daily. 
• If needed, the dosage may be raised to 1 drop per hour in the first week of treatment.  
• Discontinuing therapy prematurely should be avoided, and re-evaluation of the patient is recommended if symptoms do not improve within two days. 

For post-operative inflammation:  

• 1-2 drops of Lotemax are recommended to be administered into the conjunctival sac of the operated eye(s) four times a day, starting 24 hours after surgery, and continuing throughout the first two weeks of the post-operative period. 

Before using this eye drop, wash your hands and shake the bottle well. Avoid touching the dropper tip or letting it touch your eye or any other surface to prevent contamination. 

To administer: 

1. Tilt your head back, pull down the lower eyelid, and place 1 drop into the pouch. 
2. Close your eyes for 1 to 2 minutes and apply gentle pressure at the corner of your eye to prevent the medication from draining out. 
3. Do not blink or rub your eye. Repeat for the other eye if directed to do so and for doses greater than 1 drop. 

Avoid rinsing the dropper and remember to replace the dropper cap after each use. 

Allow a 5 to 10-minute interval before applying other eye medications if you are using a different type (e.g., drops or ointments). Administer eye drops before ointments for optimal delivery into the eye. 

For the best results, use Lotemax according to your doctor’s instructions. Do not modify the dosage or frequency of administration without your doctor’s consent, as this may heighten the likelihood of adverse effects. 

Side Effects 

Listed below are potential adverse reactions associated with the use of Lotemax. Notify your physician or pharmacist promptly if these symptoms last or worsen. 

Common Side Effects 

• Minor burning sensation during eye drop application 
• Eye pain 
• Blurred vision 
• Dry or watery eyes 
• Feeling of a foreign object in the eye 
• Light sensitivity 
• Headaches 
• Runny nose 
• Sore throat 

Serious Side Effects 

• Pain during application 
• Increasing redness or itching 
• Eye pain or swelling 
• Difficulty closing the eye 
• Pain behind the eyes 
• Abrupt changes in vision 
• Tunnel vision 
• Seeing halos around lights 
• Redness of the eye 
• Severe discomfort 
• Eye crusting or discharge 

Seek immediate medical attention if you experience any side effects listed above while using this eye drop. 

Allergic Reactions 

Severe allergic reactions to Lotemax are infrequent; however, seek medical attention immediately if you experience any symptoms listed below. 

• Rash 
• Facial, throat, or tongue swelling or itching 
• Breathing difficulties 
• Severe dizziness 

Lotemax side effects listed here are not complete. Inform your doctor promptly if any unlisted side effects occur and cause discomfort.  

Cautions 

Before starting treatment with Lotemax, inform your doctor and pharmacist of any allergies you have, including allergies to loteprednol, other medications, or any ingredients in Lotemax eye drops. You may ask for a list of the ingredients from your pharmacist. 

Before using Lotemax, disclose to your doctor your medical history, particularly of:  

• Eye infections 
• Cataracts 
• Open-angle glaucoma 
• Severe nearsightedness (myopia) 
• Diabetes 

To prevent drug interactions, inform your healthcare provider and pharmacist of all current and planned medications, including prescriptions, over-the-counter medicines, supplements, and herbal products before using Lotemax. 

Notify your doctor if you use contact lenses; they may advise you to avoid wearing them during Lotemax treatment. If wearing contact lenses is permitted, be aware that Lotemax contains benzalkonium chloride, which soft lenses can absorb. To prevent this, remove your soft contact lenses before applying the eye drops and wait for at least 10 to 15 minutes before reinserting them. 

Caution is advised when using corticosteroids like Lotemax to treat patients with a history of herpes simplex. They may lengthen the duration and worsen the severity of viral eye infections, including herpes simplex. 

Pregnant women should only use Lotemax when absolutely necessary, and they should discuss the risks and benefits with their doctor. 

The transfer of loteprednol into breast milk is uncertain. Consult your doctor before breastfeeding while using these eye drops.
